The city of Mainz had multiple plans for setting up a football club. After multiple failures, they finally founded the 1. Mainzer Fußballclub Hassia 1905 in 1905. Multiple annexures later, FSV Mainz was formally launched. It was formed as a member-owned club.

They competed in the regional leagues and won the prestigious Kreisliga Hessen.

The club failed to compete in Gauliga Sudwest, one of the new first-division leagues formed under the Third Reich. They endured multiple relegations during the Nazi era. They ended up in the Oberliga Südwest after World War II and struggled to reach the Bundesliga.

FSV Mainz appointed Klopp, who took them to the top flight for the first time in the club's history for the 2003-04 season. They played in the UEFA Cup for the first time in 2005 as well. However, they suffered relegation again before reaching the top-flight under Tuchel.

After getting promoted in 2009, they never got relegated. It was dirty relegation dogfights first before finding consistency in the previous seasons. They finished sixth in the recently concluded 2024-25 season and qualified for the UEFA Europa Conference League.
